Question about getting back on SSI for mental illness?

I was wondering how to get back on disability / SSI. I was approved and on it 7 years ago for mental illness. I dont really wanna get into details about my problems for various reasons. I was only 23 and decided maybe I could block everything out and work and try living a normal life. Its been pretty much a failed experiment , Ive had around 25 jobs since then and my mind seems to of gotten worse. , Anyways my question is how easy is it to get back on SSI? Would I have to reapply and go thru all the court things again?
