Your poll poorly reflects the belief of creationists. Even the Bible says that humans have evolved (changed over time). Creationists won't argue that. Humans used to be larger and live longer, according to the Bible.

You are also pitting two different kinRAB of beliefs. It would be more correct to compare Creation with spontaneous generation, not evolution. Evolution assumes there's already a population of living things to evolve.

Reading the wording of the poll, it seems that there is no correct answer for you.
Answer two might seem to you to be correct, but includes "less advanced forms", as if evolution means turning small, simple things into big, complex things, and that humans are "more advanced" than the things that they evolved from. That's not true at all. And it's sad that none of the extremist evolutionist atheist heathens hasn't bothered to point that out yet.

Well, if you want to get technical, it's more like 78% believe in creationism. But just 40% believe in "as-is" creationism.

Those who don't believe in ANY FORM of creationism are still massively in the minority, at 16%.
