PERIPHERAL SMEAR:
RBC : Normocytic Normochromic
WBC: Mild leucocytosis with normal differentials
Platelets: adequate
BONE MARROW: Aspirate is particulate with increased cellularity. There is a prominance of plasma cells accounting for 85% of marrow cellularity. Some of these are binucleated and 15% plasma cells showing prominent nucleoll(plasmacytoblasts). Maturing myeloid and erythroid precursors are reduced. Megakaryocytes are normal in number and morphology. Trephine imprints shows normal cellularity with normal erythroid, myeloid distribution and scattered plasma cells.
IMPRESSION: Peripheral blood and marrow morphological features are consistent with marrow plasmaytosis- Multiple Myeloma is farored.
Suggested correlation with serum protein electrophoresis and skeletal survey.
