Normally women planned on having children so it was rare if there was surprises. Also nobody really knew how it worked, so everyone thought it was just natural for their stomachs to begin getting larger, feel sick etc. Many women also commited suicide thinking they were dying. when people started noticing it happened after sex, thats when body science started improving and STD's were noticed and people became more curious.
