I often get cramp near the end of football matches. What can i do?

Once the cramp strikes, what can I do to get rid of it and carry on playing. I stretch the muscles, its usually my calves, but it won't go and i have to be substitued. Also, what can i do to prevent it occuring in the first place?

Drink water with a little salt in it. Frequently. You see professionals have a quick drink when the trainer comes on.
