Hi, I was injured at work (top shipping company) and I had 2 back surgeries. I'm in my twenties. Since having permanent restrictions and not being able to go back to my original job, my employer has been giving me the run around. Recently my work comp. claims manager said i was terminated. So, I go to work and find out i wasnt fired but my name was crossed off a bid sheet for a job that fits within my restriction. Also, I had a meeting with the companies ADA rep. and there was NO discussion about accommodations. All he said was fill out this form and i had no idea what to write.When i was on the accommodations question, he said to leave it blank if i dont know.
