can a 16 year old be tested for a learning disability?

is there a comprehensive test that will determine what disability they have. I do not want to have this test taken at school because they only test you if you are failing. I work EXTREMELY hard to keep up with the other kids. I have been in special ed. before in the past but it has become apparent that I need it. Where is a good place to go to be tested?
is there a test that tests for autism AND dyslexia
